Phillip Dean Lewis, age 47, of Eldon, died Saturday, May 20, 2006, at Eldon Nursing and Rehab. He was born in Rolla on May 10, 1959, a son of the late Harley H. and Betty Ann (Jensen) Lewis.
Those who survive him include: his wife, Frances Ann Lewis; four daughters, Lisa Humphrey of Eldon, Nicole Lewis of Jefferson City, Dana Lewis of Tebbetts and Roxy Lewis of St. Louis; one son, Thomas D. Bourbon of Catawissa; one step-daughter, Anne Kolkmeyer of Eldon; six grandchildren, Kayla Estes, Trenton Humphrey, Nautika Key, Samuel Williams, Jr., Taylor Neeley and Thomas Bourbon, Jr.; two brothers, Richard and Steve Lewis; three sisters, Caroline Nelson, Bonnie Russell and Theresa Groce.
Two brothers, Danny and Otis Lewis, also preceded him in death.
Services will be at 2:00 p.m. on Tuesday, May 23, at Rekus Funeral Home in Eldon. Pastor Randall Bunch will officiate. Interment with military honors will be in Eldon City Cemetery.
Visitation will be from 5-8 p.m. on Monday at Rekus Funeral Home in Eldon.
Memorial contributions are suggested to the Phillip Dean Lewis Memorial Fund.
